Notice of Right to Good Faith Estimate Healthcare providers are legally required to provide patients who are uninsured or self-pay with a written estimate of anticipated charges before services are rendered.

  • Comprehensive Pricing: Estimates include the primary service as well as related costs like labs, medications, and facility fees.

  • Timely Delivery: Written estimates are provided within 1–3 business days of scheduling (depending on the lead time) or within 3 business days of a general inquiry.

  • Dispute Rights: Patients may initiate a dispute resolution process if the actual billed charges exceed the Good Faith Estimate by $400 or more.

  • Transparency: In accordance with the Physician Payments Sunshine Act, data regarding payments from pharmaceutical and device manufacturers to physicians is publicly available at openpaymentsdata.cms.gov.
